We are seeking a skilled and experienced Registered Nurse for a Visiting Nurse Home Care position. The ideal candidate will have a strong background in acute nursing care and home health experience, with a focus on providing high-quality patient care in a home setting.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a clear and concise manner.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
Requirements:
- 1.5+ years of acute nursing care and home health experience.
- Must have home health experience - this is non-negotiable.
- OASIS experience noted on packet/profile - required.
- IV Certification Required/1 Year IV Experience preferred.
- Med Surg experience preferred.
- May be opening cases.
- Bachelor (or higher) degree.
- Valid Driver License required for this need.
- Must be able to work independently with minimal supervision/assistance.
Additional Requirements:
- AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO TRIO/ PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. "ANYTIME" WILL NOT BE ACCEPTED.
- NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better) DURING THE COURSE OF THIS ASSIGNMENT.

- Conforms with BHS Customer Service and Performance Standards.
- Conforms with Patient's Rights Policy.
- History of dependability in meeting demands of committed work schedule.
