Position Overview:
The Home Care Physical Therapist is responsible for assessing and creating tailored treatment plans for patients across various age groups, adjusting care strategies as necessary.

Key Responsibilities:

  • Serve as a professional role model in the provision of therapy services and as a clinical resource for colleagues and students.
  • Maintain a valid Physical Therapist (PT) License in Illinois and ensure compliance with all regulatory requirements.
  • Document patient visits meticulously to provide clear and comprehensive information for subsequent care providers.
  • Educate patients and their families on discharge planning, ensuring all necessary arrangements are made prior to discharge.
  • Collaborate with other healthcare professionals and document all interactions related to patient care.
  • Administer therapeutic treatments utilizing physical agents such as heat, cold, light, water, and electricity, along with massage therapy.
  • Evaluate and fit prosthetic and orthotic devices, recommending necessary modifications.
  • Design and implement prescribed physical therapy programs aimed at restoring function, alleviating pain, and preventing further injury.
  • Conduct thorough patient assessments, including range of motion tests and functional analyses.
  • Review referrals and medical records to determine the necessity of physical therapy interventions.
  • Conduct comprehensive assessments in patients' homes and develop individualized care plans.
  • Assess home safety and recommend equipment to enhance patient safety at home.
  • Perform wound care in accordance with established protocols and departmental policies.
