We are seeking a skilled Home Health Risk Assessment Nurse Practitioner to join our team. As a member of our team, you will have the opportunity to leverage your expertise and compassion to make a direct impact on the health and well-being of others.
The ideal candidate will have a Master's Degree in Nursing or a related field, current RN and NP licensure in the state of practice, and Board certification by the AANP, ANCC, or AACN in a Matrix-approved specialty. The successful candidate will also possess strong computer skills, excellent verbal and written communication skills, and be comfortable with frequent change.
This is a PRN/Part-Time position, offering flexible scheduling and competitive compensation. You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel to provide comprehensive health assessments, diagnosis, treatment, and education to patients in various venues, including home visits, skilled facility visits, and telehealth visits.
You will be rewarded with a generous mileage reimbursement plan, and as a valued member of our team, you will have opportunities for professional growth and development.
