We are seeking a dedicated Primary Care Physician to join our innovative network of neighborhood primary care centers. As a member of our team, you will have the opportunity to provide equitable and effective value-based healthcare to local medicare patient populations.
Job SummaryThe purpose of this role is to deliver high-quality patient care in an outpatient setting, focusing on preventive medicine and promoting healthy outcomes. Our value-based care model rewards providers for achieving excellent results, driving low hospital admissions, and delivering an exceptional patient experience.

Responsibilities- Assess and diagnose patients at our local clinics
- Oversee, direct, and administer primary care services
- Prescribe and administer pharmaceutical treatments and medication
- Maintain accurate Patient Electronic Medical Record data
- Collaborate with regional and central leadership to meet health quality goals
- Work with Practice Managers to direct and manage the center care team
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- USA work authorization
- A competitive salary of $250,000 - $350,000 per year, depending on experience
- An annual bonus based on quality metrics
- 6 weeks of paid time off, inclusive of major holidays and CME
- $5,000 Continuing Medical Education stipend
