About the Role

We are seeking a highly skilled Occupational Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality patient care and rehabilitation services to individuals in their homes.

Key Responsibilities
  • Plan of Care: Develop and implement individualized occupational therapy plans to meet the unique needs of each patient.
  • Evaluation: Conduct thorough evaluations to assess patients' functional abilities and identify areas for improvement.
  • Patient Outcomes: Utilize evidence-based practices to improve patients' range of motion, muscle strength, and coordination through passive/active exercise and activity programs.
  • Minimize Patient Risk: Educate patients and their families on preventative measures to reduce the risk of re-hospitalization.
  • Family Educator/Advocate: Provide counseling and education to patients and their families on disease management and proper use of equipment.
  • Supervision: Oversee and supervise the total care of patients provided by Certified Occupational Therapy Assistants (COTAs).
  • Communication/Collaboration: Communicate effectively with COTAs, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Policies and Regulations: Adhere to agency policies, procedures, rules, and regulations, and maintain accurate clinical documentation.
Requirements
  • Education and Licensure: Current license as an Occupational Therapist in the state of employment.
  • Experience: Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred.
  • Physical Demands: Ability to perform all physical demands of the job, including lifting, bending, and standing for extended periods.
