We are seeking a compassionate and dedicated Registered Nurse Home Health to join our team at Elara Caring. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our patients in their preferred home setting.
Key Responsibilities- Assess and evaluate patient needs, developing and implementing individualized plans of care to ensure optimal health outcomes.
- Provide comprehensive nursing care, including medication management, wound care, and education to patients and their families.
- Collaborate with the interdisciplinary team to coordinate care and services, ensuring seamless transitions and continuity of care.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
- Must be able and willing to travel 50%.
