A big-picture view of what you'll do as a Home Health Licensed Practical Nurse includes:

  • Providing home-based care to patients with illnesses, injuries and chronic diseases often after hospital discharge under the supervision of an RN
  • Working with a team of physicians, RNs, CNAs and aides to deliver quality patient care
  • Taking patient vitals, doing assessments, documenting progress and reporting changes to RN supervisor
  • Assisting RN in carrying out patients plan of care and teaching patient self-care techniques
  • Educating family on the patients plan of care, medication and home safety
  • Collaborating with interdisciplinary team to coordinate patient care and plan for discharge

To succeed in this role, you should possess:

  • Graduate of an accredited practical nursing program and active LPN license in FL
  • Minimum of 1 year home healthcare experience required
  • CPR certification/ BLS certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills and compassion
