Responsibilities:
- Coordinate patient care, ensuring continuous assessment of each patient's and family's needs while implementing the interdisciplinary care plan.
- Deliver services in accordance with physician orders and the established plan of care (POC).
- Ensure compliance with federal and state regulations in care delivery.

Qualifications:
- Must be a graduate of an accredited clinical education program.
- Must hold a valid license in the state of practice.
- Minimum of one year of experience as a licensed professional.
- Basic technology skills, including mobile device operation, are required.

Preferred Qualifications:
- Experience in Medicare home health or hospice settings is preferred.

Requirements:
- Valid state driver’s license.
- Maintain automobile liability insurance as required by law.
- Reliable transportation in good working condition.
- Ability to safely drive in various weather conditions.
- CPR certification for healthcare providers.
