We are seeking a skilled Transition Coordinator to join our team at HopeWell. As a key member of our Shared Living program, you will play a vital role in supporting individuals with intellectual disabilities to live happy, healthy, and productive lives.
Key Responsibilities- Develop and implement high-quality support plans that promote independence and well-being for individuals in our Shared Living program.
- Collaborate with our Director to recruit, select, and train caregivers, ensuring they have the necessary skills and knowledge to provide exceptional support.
- Provide on-call coverage and crisis management, ensuring the well-being and safety of individuals in our care.
- Ensure compliance with all applicable DDS regulations, policies, and procedures, and oversee the licensure and certification of caregivers.
- Build positive relationships with individuals, caregivers, and families, promoting a culture of dignity and respect.
- Bachelor's degree in a human services field or related field.
- 2+ years of experience working with individuals with intellectual disabilities, preferably in a Shared Living setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong focus on customer service and quality care.
