Care Coordinator RN

Job Summary:
Beebe Healthcare is seeking a Registered Nurse - Care Coordination to join our team. As a Care Coordinator RN, you will play a critical role in coordinating care for patients with chronic conditions, ensuring they receive the necessary support and resources to manage their health.
Responsibilities:
- Assist patients with self-management of chronic disease processes, including goal setting, teach-back method of learning, and promotion of patient advocacy.
- Coordinate healthcare services, including outside organizations, to ensure patients receive comprehensive care.
- Act as a client advocate, partnering with Community Health Workers (CHWs) to address social determinants of health barriers.
- Develop and evaluate patient plans of care, identifying areas for improvement and education.
- Monitor patient healthcare pathways, ensuring adherence to care plans and removing obstacles to desired care outcomes.
Requirements:
- Graduate of a nursing school
- Valid State of Delaware Registered Nurse (RN) license
- Minimum of two (2) years of work in the healthcare field
- BLS (CPR & AED) certification issued by the American Heart Association (AHA)
- Previous nursing experience in a Clinic/Outpatient environment
- Previous work using Electronic Health Records (EMR)
