Job Title: Registered Nurse - Care Coordination

Job Summary:

Beebe Healthcare is seeking a Registered Nurse - Care Coordination to join our team. As a Care Coordinator RN, you will play a critical role in coordinating care for patients with chronic conditions, ensuring they receive the necessary support and resources to manage their health.

Responsibilities:

  • Assist patients with self-management of chronic disease processes, including goal setting, teach-back method of learning, and promotion of patient advocacy.
  • Coordinate healthcare services, including outside organizations, to ensure patients receive comprehensive care.
  • Act as a client advocate, partnering with Community Health Workers (CHWs) to address social determinants of health barriers.
  • Develop and evaluate patient plans of care, identifying areas for improvement and education.
  • Monitor patient healthcare pathways, ensuring adherence to care plans and removing obstacles to desired care outcomes.

Requirements:

  • Graduate of a nursing school
  • Valid State of Delaware Registered Nurse (RN) license
  • Minimum of two (2) years of work in the healthcare field
  • BLS (CPR & AED) certification issued by the American Heart Association (AHA)
  • Previous nursing experience in a Clinic/Outpatient environment
  • Previous work using Electronic Health Records (EMR)
