Job Summary

We are seeking a compassionate and skilled Home Health Care Professional to join our team. As a Home Health Care Professional, you will provide personalized care and support to our clients, enabling them to maintain their independence and quality of life.

Key Responsibilities
  • Provide high-quality, personalized care and support to clients in their homes
  • Implement client care plans in accordance with agency policies and procedures
  • Participate in client case conferences and provide updates on client progress
  • Assist clients with daily living activities, such as bathing, dressing, and grooming
  • Prepare meals and snacks, and assist with light housekeeping
  • Provide emotional support and companionship to clients
  • Assist clients with medication management and administration
  • Document client progress and report any changes or concerns to the Registered Nurse
Requirements
  • High School Diploma or Equivalent
  • Home Health Aide certification
  • 1 year of experience in home health care
  • Ability to read, understand, and execute instructions
  • Ability to communicate effectively with clients and staff
  • Ability to work in a fast-paced environment and adapt to changing situations
