Job Summary:
The Rehabilitation Therapist evaluates, assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury, in accordance with current federal, state, and local standards, guidelines, and regulations that govern long-term care facilities to assure the highest quality of resident care and positive relationships among the community, team members, residents, and families.
Key Responsibilities:
- Evaluate and assess residents' physical abilities and develop personalized rehabilitation plans
- Administer and supervise rehabilitative programs, including physical therapy, occupational therapy, and speech therapy
- Collaborate with healthcare teams to ensure comprehensive care and effective communication
- Monitor and document residents' progress, adjusting treatment plans as needed
- Provide education and support to residents and their families on rehabilitation techniques and strategies
Requirements:
- Graduate of an accredited Physical Therapy program
- At least 2 years' experience as a physical therapist
- Current Physical Therapist state license
- Excellent verbal and written communication skills
