We are in search of a dedicated Licensed Practical Nurse to become a vital member of our team.

Your responsibilities will include the precise and timely administration of medications, treatments, documentation, and emergency responses as required.

You will oversee the nursing care of all assigned residents and delegate direct care tasks.

As part of the Nursing department, you will collaborate with fellow LPNs, RNs, CNAs, Dietary Aides, Physical Therapy, and Activities Staff, reporting to our Nurse Managers and Director of Nursing.



Key Responsibilities:
• Monitor and administer all treatments, including but not limited to: dressings, catheter insertion and maintenance, suctioning, oxygen, etc.
• Assist in direct resident care as needed and administer medications
• Conduct routine checks of residents' vital signs
• Maintain accurate documentation of medications administered and care provided
• Supervise and guide CNAs in accordance with the Nursing Practice Act
• Serve as the primary resource for residents and staff regarding nursing care issues
• Provide comprehensive reports on total resident care, including data collection, medications, treatments, and daily living and psychological support
• Count narcotics and sedatives, rectify any discrepancies, and document the counts appropriately
• Report medication errors or adverse reactions immediately, notifying the resident's attending physician and completing necessary documentation
• Respond promptly to all resident call lights
• Manage the admission and discharge of residents with appropriate documentation

Qualifications:
Our ideal candidate will possess 2-3 years of relevant experience and the following credentials/skills:

• Associate or Bachelor’s degree in a related field
• Strong communication skills
• Team-oriented mindset
• Excellent work ethic
• Active LPN license registered in the State of Wisconsin
• Exceptional interpersonal skills
• Ability to follow and provide instructions professionally
• Keen attention to detail
• CBRF certification or willingness to obtain within 90 days
• CPR certification required
• Empathy and compassion
