Clinical Case Manager

We are seeking a skilled Clinical Case Manager to join our team at UP Health System - Marquette. As a Clinical Case Manager, you will play a critical role in promoting continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
Key Responsibilities- Consult with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care.
- Monitor patient and family satisfaction, responding to questions and complaints regarding care.
- Participate in discharge planning, coordinating patient transfers and community resources.
- Perform chart reviews to identify issues with service delivery, patient outcomes, and satisfaction.
- Associate's degree in Nursing (Bachelor's degree preferred)
- 5 years of nursing experience (5 years of hospital experience preferred)
- Registered Nurse (RN) licensure in the State of Michigan
- Basic Life Support Health Care Provider (BLS-HCP) certification
