Position Overview

ROLE SUMMARY:

The Substance Use Care Coordinator plays a vital role within the Community Health Team at Lamoille Health Partners. This position is dedicated to collaborating with patients and their families or caregivers to help them articulate and achieve their health objectives.

In partnership with the primary care provider, the care coordinator will develop a comprehensive care strategy tailored to support the patient in reaching their desired outcomes.

This role encompasses a variety of responsibilities, including:

  • Facilitating care coordination and transitional care interventions.
  • Providing referrals to community resources.
  • Conducting brief behavioral health interventions and counseling.
  • Performing mental health and substance misuse screenings and assessments.

The aim is to cultivate a supportive therapeutic relationship with patients, guiding them towards improved health through counseling, motivation, and education.

It is essential for the care coordinator to possess a thorough understanding of substance use, the American Society of Addiction Medicine (ASAM) criteria, family dynamics, community resources, and the regulatory and legal frameworks that impact health outcomes.

This position is integral to the primary care team, delivering direct services during patient consultations and scheduled visits.
