About the Role

We are seeking a dedicated and compassionate Nurse Practitioner to join our team at Ambercare, a leading Home Health Care provider in New Mexico. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and promoting health outcomes in the comfort of patients' homes.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, psychological, social, and spiritual needs, identifying areas for intervention and developing personalized care plans.
  • Collaborate with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care and ensure seamless transitions.
  • Provide direct patient care, including therapeutic, preventative, and rehabilitative nursing procedures, as directed by the attending physician.
  • Maintain accurate and timely documentation of patient records, charts, progress notes, and reports.
  • Participate in ongoing education and professional growth, staying up-to-date with best practices and industry developments.
Requirements
  • Graduate of an accredited registered nursing program and licensed in the state.
  • At least one year of clinical practical nursing experience, preferably in intensive care, rehabilitation, or medical surgical nursing.
  • Valid CPR certification and reliable transportation.
