As a Sacred Heart Home Health team member, you will have the opportunity to make a meaningful difference in the lives of our patients. We are seeking an experienced Occupational Therapist to join our team.

Job Summary:

The occupational therapist contracted through Sacred Heart Home Health is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Duties & Responsibilities:
  • Assume responsibility for ongoing interdisciplinary assessment and development of individualized plan of care in partnership with patient, representative (if any), and caregiver(s).
  • Provide services ordered by physician as indicated in the plan of care.
  • Provide patient, caregiver, and family counseling.
  • Provide patient and caregiver education.
  • Communicate with physician responsible for home health plan of care and other healthcare practitioners related to current home health plan of care.
  • Evaluate patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.). Evaluate home environment for hazards or barriers to more independent living. Identify equipment needs. Participate in development of total plan of care.
  • For patients who plan to return to work, occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Sacred Heart Home Health policy.
  • Develop treatment program and establish goals for improved function.
  • MAY teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Sacred Heart Home Health policy.
  • MAY fabricate splints and instruct patient in use of various types of adaptive equipment to improve function.
  • MAY train patient in use of prosthetic and/or orthotic devices.
  • Maintain appropriate clinical records, clinical notes, and reports to physician any changes in patient's condition. Submit these records in accordance with Sacred Heart Home Health policy.
  • Participate and contribute in HHA's quality assessment and performance improvement program and HHA-sponsored in-service training, case conferences, and other meetings as required by Sacred Heart Home Health policy to ensure coordinated and comprehensive plans of care for patients of Sacred Heart Home Health.
  • Identify patient and family/caregiver needs for other home health services and refer as appropriate.
  • Instruct patient's family/caregiver and other Sacred Heart Home Health health care personnel in patient's treatment regime when indicated and appropriate.
  • Supervise Certified Occupational Therapy Assistants according to Sacred Heart Home Health policy and state regulations.
  • When therapy is only skilled service, instruct, supervise and evaluate home health aide care every two (2) weeks.
  • Participate in discharge planning for patient.

 

Job Requirements:Education:
  • Must be licensed where HHA is located if licensure is required.
Experience:
  • Two years of clinical experience as an occupational therapist.
Knowledge, Skills & Abilities:
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR Certification.

 $120,000 - $150,000 per year, based on experience and qualifications. Must be a licensed driver with an automobile that is insured in accordance with state or Sacred Heart Home Health requirements and is in good working order.
