Position Overview

The LPN provides direct personal care services to residents and ensures adherence to all company policies, procedures, standards, and residents' rights during their assignment.

Key Responsibilities
  • Follow the Resident Assessment and Care Plan for each resident.
  • Deliver skilled personal assistance with bathing, dressing, oral hygiene, grooming, and other necessary support to meet all personal needs promptly.
  • Provide respectful assistance for residents with incontinence.
  • Administer medications in accordance with state regulations and training standards, ensuring that all medication records are current.
  • Maintain an accurate count of narcotics, reporting any discrepancies to the Nursing Supervisor immediately.
  • Assess situations within your professional scope (e.g., falls, skin tears, wounds) and communicate with residents' DPOA/Emergency Contacts and physicians. Document findings and report to the Nursing Supervisor or Operator as necessary.
  • Ensure all resident care logs, charts, and records are up to date.
  • Stay informed about resident behavior issues, medication effects, and any significant changes in behavior, appetite, or sleep patterns. Document notable events in nursing notes and notify the On Call or Nursing Supervisor as needed.
  • Coordinate with CMAs regarding medication changes and with CNAs on resident care updates.
  • Maintain a clean, organized, and safe home environment, including regular laundry duties.
  • Prepare meals according to menu plans, accommodating special dietary needs.
  • Engage residents in focused individual interactions and activities as per their Assessment and Care Plan, ensuring appropriate movement, exercise, and mental stimulation.
  • Stay knowledgeable about KDADS care regulations and compliance requirements.
  • Interact professionally with residents, families, and staff.
  • Participate in all mandatory staff education meetings.
  • Perform additional reasonable duties as assigned by management.
  • Monitor the condition of the property, including yard maintenance and report any issues to management.
  • Act in the best interest of Home of the Flint Hills at all times.
  • Adhere to company appearance guidelines while working or representing the organization.
  • Become familiar with all company policies and procedures.
Qualifications:

Must be currently certified and in good standing as a Licensed Practical Nurse or Registered Nurse by the State of Kansas. Successful completion of the organization's Orientation and Training Programs is required.

Working Conditions / Physical Requirements
  • Ability to lift heavy objects with assistance.
  • Frequent bending, squatting, kneeling, and twisting.
  • Repetitive motions, including cleaning tasks.
  • Ability to push or pull a minimum of 50 pounds.
  • Standing for extended periods (minimum of 1 hour) for various tasks.
