Position Summary: The role involves conducting thorough reviews of medication orders, ensuring accuracy in verification and dispensing. Engage in medication reconciliation, provide clinical consultations, and adhere to established pharmacy management protocols. Collaborate with healthcare professionals through chart assessments and act as a vital link between the pharmacy and other medical disciplines, including physicians.
Support the pharmacy director in enhancing operational processes and contribute to monitoring adverse events and medication errors, focusing on patient safety. Depending on staffing requirements, you may participate in an interdisciplinary team within a decentralized unit-based model and engage in collaborative rounds.
Key Responsibilities:
- Comply with all hospital and departmental regulations and procedures.
- Interpret and evaluate physician prescriptions and medication directives.
- Serve as a reliable drug information resource for patients and healthcare staff.
- Compound and dispense medications and pharmaceuticals essential for patient care.
- Coordinate clinical pharmacy services to ensure optimal patient outcomes across all demographics, including neonatal, pediatric, adult, and geriatric populations.
- Facilitate the establishment and maintenance of programs that promote effective and safe drug therapies.
- Conduct inspections and monitoring across all hospital units, ensuring compliance with regulations and standards.
- Perform additional duties as assigned.
Requirements:
- Valid Pharmacist certification or licensure in the State of Texas.
- Doctorate in Pharmacy or a Bachelor of Science degree or equivalent qualification.
- Graduated from an accredited School of Pharmacy; PharmD preferred.
- Experience in managing care for all age groups, from newborns to the elderly, either through education or training.
- IV certification to be obtained within 90 days of employment.
- Experience with Meditech systems is preferred.
Skills and Competencies:
- Ability to work effectively within an interdisciplinary team.
- Proficient in medication order review, verification, and dispensing.
- Skilled in executing medication reconciliation and clinical consultations.
- Participate in chart reviews and multidisciplinary rounds, serving as a liaison with other healthcare professionals.
- Assist in process improvement initiatives and other projects as directed by the pharmacy director.
- Contribute to monitoring adverse events and medication safety protocols.
- Flexible scheduling is required, including weekends, holidays, and on-call duties.
Language Proficiency:
- Ability to read and communicate effectively in English; proficiency in additional languages is preferred.
Technical Skills:
- Basic computer literacy.
- Familiarity with automated pharmacy systems and IV Laminar Flow Hood operations.
