About the Role

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Enhabit Home Health & Hospice. As a national leader in home-based care, we are committed to expanding what's possible for patient care in the home.

Key Responsibilities
  • Perform visits and coordinate client care, serving as the point of contact for all disciplines involved in patient care.
  • Oversee the frequency of visits for each episode and consult with physicians and office staff as needed.
  • Collect accurate OASIS data, maintain current medication profiles, and ensure timely reporting of lab values to physicians.
  • Attend weekly case conferences and monthly case manager meetings to discuss patient care and best practices.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • A minimum of one year of clinical experience, preferably in home health care.
  • Demonstrated knowledge and skill in current nursing practice.
  • A valid state driver's license and automobile liability insurance.
