Physical Therapist (Per Diem) at Sky View Rehabilitation and Health Care

Description:

We are seeking a highly skilled Physical Therapist to join our team at Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility located in New York. As a Physical Therapist, you will play a vital role in helping our residents achieve their highest level of physical, mental, and social well-being.

Responsibilities:
  • Assist in planning, organizing, developing, and implementing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations.
  • Plan, develop, organize, evaluate, and implement the programs, goals, objectives, policies, and procedures of the physical therapy program.
  • Interpret and/or instruct in physical therapy policies and procedures to personnel, residents, family members, etc. as necessary.
  • Participate in discharge planning, development, and implementation of resident care plans, resident assessments (MDS), etc. as necessary.
  • Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
  • Maintain treatment records, resident files, and progress notes as required and maintain an adequate liaison with families and residents.
Requirements:
  • Must possess a current license and registration to practice as a Physical Therapist in the State of New York.
  • Must possess current malpractice insurance, license in good standing.
  • Dedicated to providing extraordinary compassionate care as a Physical Therapist.
  • Energetic, enthusiastic, committed, and passionate about quality care.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
  • Patient, cheerful disposition, and willingness to assist any residents and co-workers.
  • Excellent communication, customer service, and organization skills.
  • Trusted to work independently and in a team environment.
  • Able to work well under pressure, flexible, and will work harmoniously with all staff.
  • Covid-19 vaccinations are encouraged but not mandated.
