Speech Therapist Job Description

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Speech Therapist to join our team of skilled home healthcare providers.

Job Summary:

The Speech Therapist will provide Speech Therapy services to patients in their homes, working under the direction of a physician and nursing supervisor. The ideal candidate will have a strong commitment to providing quality care and a passion for helping others.

Responsibilities:

  • Evaluate patients' level of functioning and hearing, and recommend mechanisms to enhance their hearing ability.
  • Provide initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish a Speech Therapy treatment plan in consultation with the physician and revise it with the physician's approval as necessary.
  • Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
  • Communicate with other team members and instruct them in speech pathology techniques as needed.
  • Teach, supervise, and counsel patients and their families in the total Speech Therapy program and other related problems.
  • Record all evaluation data, treatments, and patients' responses to therapeutic intervention in a timely manner.
  • Supervise Home Health Aides every 14 days when Speech Therapy is the only professional discipline involved in the patient's care.
  • Record and report to the physician patients' reactions to treatment and any changes in their condition.

Qualifications:

  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
