Key Responsibilities- Diabetes Education and Management: Provide individualized diabetes self-management education/training and medical nutrition therapy to patients, integrating with their medical goals.
- Collaboration and Communication: Work cooperatively with healthcare providers and other team members to promote holistic care for patients and families.
- Program Development and Evaluation: Contribute to the development of training materials and participate in a continuous quality improvement process to measure program effectiveness and identify opportunities for improvement.
- Patient Assessment and Follow-up: Collect and analyze patient assessment data, develop educational and care management goals, and conduct follow-up assessments to evaluate the effectiveness of care management services.
- Certification and Experience: Certified Diabetes Care and Education Specialist (CDCES) with at least three years of experience in diabetes care, education, and program management.
- Knowledge and Skills: Working experience with insulin pumps and continuous glucose monitoring technology, knowledge of health insurance coverage and system processes, and prior experience in DSMES Program management with ADA ERP or ADCES Education Program.
