Patient Care Specialist

As a Speech Language Pathologist at CHRISTUS Health, you will play a vital role in ensuring our patients receive the highest quality care. Your primary responsibility will be to accurately interpret referrals and conduct thorough assessments of patients. This will involve gathering and interpreting various data elements from medical records, other care providers, and patients and their families. Your goal will be to develop an effective plan of care (POC) that achieves the desired clinical outcomes.

Key Responsibilities
  • Proactively communicate with other clinical staff when the presentation of the patient exceeds one's skill, knowledge, or understanding of the patient's condition.
  • Implement the POC that attains expected clinical outcomes.
  • Understand the parameters of patient's condition each visit and adjust behaviors accordingly.
  • Complete all documentation thoroughly and charges accurately the same day treatment is rendered.
  • Develop in conjunction with patient and family a set of realistic, reasonable goals with associated time frames and frequencies.
  • Communicate effectively the POC to delegee's of care in a manner that is easily understood and easily translated into action.
  • Monitor the established POC and goals and modifies them as indicated based on the patient's response to treatment.
  • Utilize input from supervised staff and other care providers modifying POC or treatment according to patient's condition.
  • Recognize cognitive, physiological, physical and functional changes occurring with the patient and modifies POC and/or communicates with other care providers as indicated.
  • Proactively communicate in a timely manner with the patient's physician and or nursing staff when changes in orders are appropriate.
  • Monitor the results of the communication to ensure that the desired outcome is achieved.
  • Establish and maintain patient and family relationships to maximize clinical outcomes and eventual disposition.
  • Actively plan for the patients discharge from the day of admission through the entire length of stay demonstrated in patient documentation and participation in meetings or discussions with other care providers.
  • Efficiently utilize the time and skills of supervised staff that maximizes productivity.
  • Ability to observe and evaluate treatment effect, recommends change to physician as needed.
  • Ability to perform an appropriate assessment on all patients as related to the therapy requested and provided and reassessments as her policy.
  • Able to assess patient pain interfering with optimal level of function or participation in rehabilitation' makes appropriate physician contact for intervention.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Formulates and implements a teaching/education plan based upon identified learning needs of the patient and family and evaluates effectiveness of learning on an ongoing basis.
  • Demonstrates knowledge of physical therapy modalities.
  • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
  • Manages and operates equipment safely and correctly. Maintains department cleanliness and safety.
  • Communicates appropriately and clearly with physicians and other members of the patient team.
  • Works with Case Management to facilitate discharge planning for patients.
  • Participates in coordination and implementing patient care to ensure the patient's needs are met and hospital policy is followed.
  • Maintains records pertinent to personnel and operation of the department.
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Utilizes time in a productive manner and emphasizes patient care and by-products related to delivering that care.
  • Coordinates scheduling of patients.
  • Instruct, train, and provide consultation to patient and family members.
  • No more than five complaints received about the employee during the year.
  • Performs an acceptable quantity of patient treatments daily.
  • Offers assistance to fellow staff members when work is done.
  • Complete evaluations on day patient assessed, including the long term and short term goals.
  • Progress reports on all patients with be completed within the specified time frame.
