Clinical Pharmacist Practitioner

We are seeking a highly skilled Clinical Pharmacist Practitioner to join our team at the Veterans Affairs, Veterans Health Administration. As a Clinical Pharmacist Practitioner, you will play a critical role in providing high-quality patient care and contributing to the mission of the Pharmacy Service.
Key Responsibilities- Design, implement, and assess therapeutic plans for patients with endocrinology and health failure conditions.
- Collaborate with interdisciplinary teams to optimize medication therapy and improve patient outcomes.
- Conduct physical assessments, order laboratory tests, and interpret diagnostic results to inform medication therapy decisions.
- Monitor and assess patient outcomes, managing ongoing therapy or transitioning to other levels of treatment as needed.
- Participate in team huddles, rounds, and meetings to ensure seamless communication and coordination of care.
- Develop and implement evidence-based treatment protocols, criteria for use, and new medication requests.
- Contribute to the development and conduction of resident research projects, including research question development, methodology, and presentation.
- Design, conduct, and publish original research in refereed journals or as abstracts in scientific meetings.
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
- Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
- Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
- Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
This is a full-time position with a work schedule of Monday through Friday. Telework and virtual work arrangements are not available for this position.
