About the Role

Maximus is seeking a skilled Field Registered Nurse Assessor to provide high-quality home assessment services to vulnerable populations in the Bronx, New York. As a key member of our team, you will conduct assessments to determine eligibility for community-based long-term services, working closely with our clinical leadership team to ensure accurate and timely evaluations.

Key Responsibilities
  • Travel to consumer homes, facilities, or conduct assessments through telehealth modality, as needed.
  • Perform in-home, in-facility, or telehealth assessments using the UAS-NY assessment tool, evaluating individual health status, strengths, care needs, and preferences.
  • Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  • Regularly review medical documentation, reporting any concerns, conflicting information, or issues that may arise during the evaluation process.

Requirements

Preferred Qualifications
  • Prior experience using the UAS-NY Community assessment tool.
