About the Role

The Hospice Social Worker position is a full-time opportunity with a comprehensive benefits package, including a 401(k) with match, paid holidays, paid time off, medical, dental, and vision insurance, short and long-term disability insurance, life insurance, and continuing education funds. Additionally, this role includes a monthly cell phone stipend and mileage reimbursement.

Key Responsibilities
  • Complete admission and transfer processes with patients and families, explaining hospice services and insurance benefits, and completing necessary paperwork.
  • Provide counseling and education to patients and families regarding the psychological, emotional, and social aspects of death and dying.
  • Assist patients and families in identifying and utilizing resources to meet environmental, financial, legal, or psychological needs.
  • Document all patient/family contact and activity in a timely and professional manner.
Requirements
  • A Master's degree in social work from a Council on Social Work Education-accredited school, with current Michigan Social Work licensure as a clinical social worker.
  • A current Michigan driver's license and available transportation.
  • Sound clinical judgment and the ability to work independently.
  • Knowledge of and/or experience with end-of-life care is preferred.
  • The ability to communicate effectively both verbally and in writing through concise charting.
