About the Role

We are seeking a skilled Advanced Practice Clinician to join our Housecalls team at UnitedHealth Group. As a key member of our team, you will play a vital role in providing high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.
Requirements
  • Active NP license or ability to obtain by start date, with unencumbered licensure.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and sitting, standing, and kneeling for physical assessments.
  • Reliable transportation for home visit assessments.
Preferred Qualifications
  • May be requested to obtain additional licensure in other geographic areas.
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
