Job Title: RN Case Manager Trainee

The RN Case Manager Trainee will work collaboratively with their mentor to develop the skills necessary to provide high-quality, patient-centered care to customer-owners. This role will focus on delivering timely and beneficial care that promotes quality and cost-effective health care outcomes, while adhering to established standards for case management practice and Advance Access principles.

Key Responsibilities:
  • Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of customer-owners.
  • Conduct thorough assessments of customer-owners to identify their health needs and develop strategies to address them.
  • Provide education and support to customer-owners to promote healthy behaviors and self-management of their health conditions.
  • Work closely with the healthcare team to ensure seamless transitions of care and minimize hospital readmissions.
  • Document all interactions with customer-owners and the healthcare team in a timely and accurate manner.
Requirements:
  • Current registered nurse license in the State of Alaska.
  • Basic Life Support (BLS) certification is required; OR ability to obtain within 30 days of hire.
  • Compliance with SCF's Employee Health Procedure is a condition of employment.
  • Documentation of completed immunizations prior to first day of employment, including MMR, Varicella, Hepatitis B, Influenza, T-dap, and COVID-19 vaccination.
