Rehabilitation Specialist

We are seeking a skilled Rehabilitation Specialist to join our team, providing assistance to clients in gaining skills based on their individualized goals.
Key Responsibilities:- Provide rehabilitation services to clients, including medication management, life skills development, and recreational activities.
- Participate in weekly team meetings to coordinate client care and develop person-centered care plans.
- Meet organizational standards in timely and accurate documentation, focusing on client progress and treatment.
- Work with clients in the community and in the office, with a primary focus on community-based services.
- Bachelor's degree from a four-year college or university, preferably in psychology, social work, or a related field.
- Experience working and/or volunteering in the behavioral health, criminal justice, or social services field.
- Driver's license required.
