At Option Care Health, we are seeking a talented Clinical Pharmacist to join our team in providing safe and appropriate pharmacy services to our patients in the comfort of their own homes.



Job Responsibilities:


  • Assess Patient Suitability: Determine the suitability of individual patients for home care, collecting and organizing all patient-specific information needed to make an informed decision.
  • Design and Implement Pharmacy Care Plans: Develop, recommend, monitor, and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
  • Provide Medication-Use Education: Educate Option Care staff, other professionals, home care patients, and their caregivers on proper medication use.
  • Manage Perpetual Inventory Systems: Comply with state/federal laws and USP standards, including DEA controlled substance regulations, and manage perpetual inventory systems for schedule II narcotics and other medications subject to diversion.
  • Participate in Quality Assurance and Improvement: Participate in the process for assessing, managing, and reporting medication-related incidents and equipment errors.


Requirements:


  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • At least one year of experience in sterile IV admixture procedures or knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).


Preferred Qualifications:


  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Experience in home care, hospital, or other clean room setting.
