LPN Patient Educator

The Patient Educator LPN plays a vital role in improving health outcomes for patients with chronic diseases. As a member of our interdisciplinary team, you will work closely with patients, healthcare providers, and other stakeholders to coordinate care, educate patients, and enhance communication and continuity of care.
Key Responsibilities- Act as a patient advocate, identifying and communicating opportunities for care interventions and addressing deficits and gaps in care.
- Coordinate care services for patients with chronic diseases, such as diabetes, hypertension, and asthma, in accordance with established clinic policies and procedures.
- Implement targeted strategies to improve health, functional, or quality of life outcomes, including disease management and patient teaching plans.
- Monitor hospitalizations, emergency or urgent visits, institutional facility admissions, and re-admissions to identify issues and implement follow-up for improved outcomes.
- Coordinate and facilitate patient education groups and participate in patient care coordination, pre-visit planning, huddles, one-on-one communication, email, patient contact, and documentation of any services accessed and/or health care-related concerns/needs that have arisen since last visit.
- High school diploma or equivalent required; completion of an accredited Licensed Practical Nursing program or RN program; Active Florida LPN or RN license.
- Minimum of two (2) years' experience as a Licensed LPN or RN in Health Care or Managed Care is preferred.
- Practical experience in a patient educator role or evidence of training in patient education.
- Knowledge of health implications of chronic diseases and risks.
- Excellent skills in oral and written communications skills.
- High level of initiative and leadership.
- Good advocacy skills.
- Must have proven ability in handling multiple projects and setting priorities, as well as effective time management.
