LPN-Acute Medical Physical Rehab Nurse

St. Mary's Health Care is seeking a skilled LPN or Registered Nurse to join our team in the Acute Medical Physical Rehab department. As a member of our team, you will contribute to the assessment of patients, administer medications, and educate patients and families about disease treatment plans.
Responsibilities:
• Contribute to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).
• Administer medications and observe patients for adverse reactions to medications or treatments.
• Perform routine laboratory tests and simple therapeutic services.
• Educate patients/families about disease treatment plan including self-care post discharge, holistic health needs, available resources and follow up care.
• Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
Licensure / Certification / Registration:
• LPN Required Credential(s):
• BLS Provider obtained within 3 Months (90 days) of hire date or job transfer date.
• Licensed Practical/Vocational Nurse credentialed from the New York Board of Nursing. Must be able to speak, read, write and follow instructions in English.
• RN Required Credential(s):
• Diploma from an accredited school/college of nursing required.
• Registered Nurse credentialed from the New York Board of Nursing. Must be able to speak, read, write and follow instructions in English.
Complexity of Work:
• Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.
Personal Protective Equipment:
• Follows Standard Precautions using personal protective equipment as required.
