Advocate Aurora Health is seeking a skilled Physical Therapist to join our Home Health team in the Far Southeast Suburbs of Illinois.
Job Summary:We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to patients in their homes. As a member of our Home Health team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that promote optimal health and well-being.
Responsibilities:- Assess patients' physical and functional abilities to develop and implement effective care plans
- Provide one-on-one therapy sessions to patients in their homes, focusing on improving mobility, strength, and functional abilities
- Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and communication
- Document patient progress and outcomes in a timely and accurate manner
- Participate in ongoing education and professional development to stay current with best practices and industry developments
- Licensed Physical Therapist in the State of Illinois
- 1 year of clinical experience or demonstration of strong clinical affiliation experience
- Basic knowledge of computer use, including Microsoft Word and email access
- Ability to learn and use computer-based scheduling and documentation systems
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Competitive annual base salary: $86,320.00 - $129,376.00
