About the Role

We are seeking a skilled RN Infusion Liaison to join our team at Providence Home Services Oregon. As a key member of our infusion services team, you will play a vital role in providing direct nursing services to patients in the hospital, home, clinic, or alternate living site.

Key Responsibilities:

  • Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
  • Deliver infusion nursing care in the home or clinic, with a focus on patient-centered care and education.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure optimal care outcomes.

Requirements:

  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Must be licensed in states wherever care is delivered upon hire.

Preferred Qualifications:

  • 1 year of experience with acute hospital care, infusion, or home health.
