We are seeking a skilled Pharmacist to join our team in the Blood and Marrow Transplant (BMT) department at LDS Hospital. As a Pharmacist in this role, you will play a critical part in ensuring the safe and effective use of medications for our patients.
Responsibilities- Provide medication therapy management for patients undergoing BMT, including ordering and dispensing medications, monitoring for adverse effects, and making adjustments as needed.
- Collaborate with the healthcare team to develop and implement individualized treatment plans for patients.
- Participate in rounds and multidisciplinary meetings to discuss patient care and treatment plans.
- Stay up-to-date with the latest research and guidelines in BMT and pharmacology, and apply this knowledge to improve patient care.
- Basic Life Support (BLS) Certification
- Advanced Cardiovascular Life Support (ACLS) Certification
- Pediatric Advanced Life Support (PALS) Certification
