Job Summary and Qualifications
Licensed Practical Nurses (LPNs) deliver direct patient care under the supervision of Registered Nurses, collaborating with the nursing team to plan, implement, and evaluate nursing care.

LPNs are responsible for observing, monitoring, and reporting patient symptoms, reactions, and progress to the healthcare team.


Key Responsibilities:
- Adhere to the facility's Standards of Performance, ICARE values, and Patient Promise.

- Monitor, document, and report changes in patients' conditions, notifying RNs as necessary.

- Maintain accurate and detailed patient records.


- Collaborate with healthcare team members to assess, plan, implement, and evaluate care plans based on patient responses and conditions.


- Perform nursing tasks safely under the indirect supervision of RNs, providing individualized education for patients, families, and caregivers as directed.

- Ensure compliance with safety and infection control standards in patient care delivery.

- Demonstrate thorough knowledge of and adherence to all policies and procedures related to job responsibilities.

- Undertake additional duties and special projects as assigned by RNs, Nurse Managers, or Directors.

Qualifications:
- Strong interpersonal skills for effective interaction with internal and external stakeholders. - Graduate of an accredited School of Practical Nursing.
  • Current licensure by the Tennessee Board of Nursing.
  • Current BLS Certification.
  • Some departments may require completion of an accredited ADN or BSN program within specified timeframes from the date of hire.
