As a Home Health Registered Nurse, your responsibilities will include:

* Providing home-based care to patients with a range of illnesses, injuries, and chronic conditions
* Working closely with a team of physicians and other healthcare professionals to develop and implement patient care plans
* Examining patients, conducting assessments, and documenting progress
* Educating patients and their families on their plan of care, medication, therapy, diet, and exercise
* Coaching family members on patient safety and home care
* Ensuring patient goals are met and coordinating discharge from services

To be successful in this role, you'll need:

* A graduate degree from an accredited nursing program and an active RN license in Montana
* At least one year of RN experience, ideally in home healthcare
* CPR certification
* Knowledge of state and federal home health regulations
* Strong clinical skills, excellent communication skills, and a compassionate approach to patient care
