Physical Therapist

We are seeking a skilled Physical Therapist to join our team. As a Physical Therapist with The Roads Home Health, you will be responsible for providing skilled therapy interventions to patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.

• Conduct thorough assessments of patients to identify their strengths, weaknesses, and goals
• Develop and implement individualized plans of care that address the patient's physical, emotional, and social needs
• Provide skilled therapy interventions, including physical therapy, to patients in their homes
• Collaborate with other healthcare professionals to ensure seamless care coordination and communication
• Document patient progress and outcomes in a timely and accurate manner
• Participate in ongoing education and training to stay current with best practices and industry developments

• Current Physical Therapy License
• 2 years of experience as a Physical Therapist, preferably in home health
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Ability to travel to patient homes and work in a fast-paced environment

• Competitive hourly rate: $70.00 per hour
