About the Role: We are seeking a highly skilled RN Case Manager to join our team at Tufts Medicine Care at Home. As a member of our interdisciplinary team, you will be responsible for managing patient care in a hospice setting, ensuring quality nursing care and effective coordination with the IDT.

Key Responsibilities:

  • Provide hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes.
  • Coordinate care in a cost-efficient manner.
  • Collaborate effectively with other members of the IDT.
  • Utilize the nursing process and current standards of nursing practice in providing hospice care.
  • Remain competent in technical nursing skills (i.e. IVs, Gtubes, etc.).
  • Provide effective patient and family teaching.
  • Communicate with physician and Tufts Medicine Care at Home staff regarding changes in patient's condition.

Requirements:

  • Massachusetts RN Licensure
  • New Hampshire RN Licensure
