Job Summary

The Telehealth LPN functions as a nursing team member under the direction and guidance of the RN or Provider. The LPN is responsible for delivering safe physical and emotional care to Veterans in a variety of settings, including outpatient primary care and specialty care clinics, procedural areas, and in the home.

Responsibilities
  • Performs appropriate screening and assists patients in accessing appropriate services to address needs.
  • Performs telehealth imaging, including tele-retinal screening, tele-dermatology, tele-wound care imaging, and other telehealth programming as technology becomes available.
  • Provides nursing care to patients, including rooming patients, obtaining weight, height, and vital signs, completing clinical reminders, documenting medication review, catheterizing, applying hot/cold compresses, applying sterile dressing, performing soaks, trimming nails, performing ear lavages, scanning bladders, performing glucometer control tests, quality control checks, immunizations, injections, and oxygen, and suture/staple removal.
  • Assists RN and/or Provider with special procedures by preparing patients, participating in time-out procedures, securing clean/sterile supplies/equipment as appropriate, assisting in collecting and labeling specimens, peri-procedural monitoring, and handling and cleaning instruments as necessary.
  • Customizes patient education to patients and family members and identifies learning needs.
  • Documents collected data or current observations in electronic medical records.
  • Assists in the coordination of patient care activities with other departments, services, and facilities (both VA and non-VA).
Requirements
  • Must be a U.S. Citizen.
  • Must be proficient in written and spoken English.
  • Must pass pre-employment physical examination.
  • Must participate in the seasonal influenza vaccination program.
  • Must participate in the Coronavirus Disease 2019 (COVID-19) vaccination program.
  • Basic Life Support (BLS) certification required.
Education and Licensure

Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or the Accreditation Commission for Education in Nursing (Formerly: National League for Nursing Accrediting Commission).

Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
