Key Responsibilities:
- Participant Assessment: Conduct evaluations to determine rehabilitation needs in accordance with APTA standards.
- Collaborative Care: Administer physical therapy interventions, including modality treatments, therapeutic exercises, and mobility training.
- Documentation: Maintain comprehensive records of participant Plans of Care (POCs) and liaise with other services as necessary.
- Effective Communication: Interact with staff, family members, and caregivers to ensure optimal care and support.
- Physical Therapy Interventions: Deliver a variety of treatments such as modality therapy, therapeutic exercises, and functional mobility training.
Qualifications:
- Education: Bachelor’s degree in a physical therapy-related discipline; completion of an APTA-accredited physical therapy program is required.
- Licensure: Must possess a valid Physical Therapist license in good standing.
- Experience: A minimum of one year of physical therapy experience in a geriatric setting is required; two years is preferred.
- Skills: Strong verbal and written communication abilities, capacity to interpret diverse instructions, knowledge of occupational therapy and speech pathology, and proficiency in budget management and report compilation.
- Bilingual: Proficiency in English and Spanish is highly preferred.
