Licensed Practical Nurse

The Licensed Practical Nurse (LPN) serves as a vital member of the Interdisciplinary Care Team (IDT) at Elwyn. This role involves assisting with the assessment, planning, and implementation of nursing services to ensure the best possible outcomes for our clients.
Key Responsibilities- Conduct comprehensive nursing assessments upon admission and as needed to identify client health needs.
- Develop and implement individualized health care plans in collaboration with the IDT.
- Administer medications, treatments, and enteral feedings as ordered by physicians.
- Monitor client physical and psychological status, reporting any changes or concerns to the IDT.
- Facilitate medical follow-up as needed and provide education to the IDT on client health care needs.
- Collaborate with Day and Work Services to ensure client health needs are communicated and documented.
- Refer clients to physicians for further medical evaluation when necessary.
- Prepare and administer medications as ordered, maintaining accurate records and documentation.
- Administer and document prescribed medications and treatments, ensuring client privacy and confidentiality.
- Provide education to direct care staff on treatment administration and documentation.
- Report any observed medication and treatment side effects to the Health Care Coordinator/Charge Nurse.
- Ensure timely reporting of injuries or occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
- Adhere to Elwyn's policies and procedures related to HIPAA and the disclosure of health protected information.
- Complete all mandatory educational requirements and obtain in-service training as required.
- Licensed as a Practical Nurse in the Commonwealth of Pennsylvania.
- One year of general nursing or medical surgical experience required; Developmental Disabilities nursing experience preferred.
- Current CPR certification.
- Demonstrated skill using an electronic medical record system.
- Excellent planning, communication, customer interaction, collaboration, and documentation skills.
- Ability to work effectively as part of a team and manage multiple priorities.
- Strong attention to detail and time management skills.
- Ability to anticipate future needed changes or identify problem areas and take effective actions.
The Licensed Practical Nurse will work in a fast-paced environment, interacting with clients, families, and healthcare professionals. The role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
