Job Description

Horizon Services, Inc. is a nonprofit organization dedicated to providing addiction treatment and reducing substance use-related challenges for individuals from diverse backgrounds. As a Lead Nurse Coordinator for our Sobering Center, you will play a crucial role in ensuring the well-being of our clients.

Key Responsibilities:
  • Screen participants for medical and mental health issues, and refer them for definitive care when necessary.
  • Monitor vitals, perform health screenings, and assess client needs for sobering.
  • Document client information, including medication records and case notes.
  • Provide a welcoming environment for participants and respond to crisis situations effectively.
  • Collaborate with the multi-disciplinary team to ensure seamless care and support.
Requirements:
  • Current Licensed Vocational Nurse (LVN) certification.
  • 1-year experience in a similar setting with demonstrated leadership skills.
  • CPR/First Aid certification.
  • Valid California driver's license and good driving record.
