Job Summary:
The Inpatient Case Manager at Facility SINAI HOSPITAL plays a critical role in promoting patient and family wellness, improving care outcomes, and ensuring access to appropriate hospital and community resources for patients with complex health needs.
Key Responsibilities:
- Collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention.
- Develop and implement effective care plans to address the unique needs of high-risk patients.
- Coordinate with hospital and community resources to ensure seamless transitions and optimal patient outcomes.
- Provide education and support to patients and families to promote self-advocacy and empowerment.
- Participate in multidisciplinary teams to identify and address system-level barriers to care.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- 2+ years of experience in hospital social work or community social services agencies, including post-graduate internship placement and/or related experiences.
- LMSW required; LCSW/LCSW-C preferred.
- MD Social Work License per level of education.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
