Key Responsibilities:
Summary Evaluate the health concerns and requirements of individuals served, create and execute nursing care strategies, and maintain accurate medical documentation.
Provide nursing services. Counsel individuals served and team members on health preservation and illness prevention. Oversee case management. Core Expectations Deliver superior outcomes through operational excellence, fulfilling daily, weekly, monthly, quarterly, and annual goals for each individual served.Cultivate and sustain a robust, dedicated team.
Propel and achieve profitable growth. Essential Duties Include: Review daily appointment requirements: ensure packets are completed and accessible to staff.
Examine daily nursing schedules, arrange, and attend when feasible. Ensure adequate coverage.
Supervise staff as necessary (e.g., medication administration, meals {modified texture, therapeutic}, activities of daily living, execution of care plans, and interactions between staff and clients).
Analyze incident reports – provide evaluations and follow-up.
Review findings and recommendations from appointments. Implement changes as needed. Document services rendered. Assess LPN notes.
Authorize orders for prescribed medications/treatments (QuickMar).
Monitor health status/vital signs/seizure disorders.
Address acute and ongoing health concerns, including documentation of "Change of Condition".
Attend Individualized Program Plans (IPP) as scheduled.
Verify Medication Administration Records (MAR) for thoroughness and accuracy.
Provide on-call services. Manage resources effectively.
Conduct PRN Assessments:
Assess falls risk, skin integrity, and choking.
Review IPP schedules, submit assessments two weeks prior to IPP.
Check follow-up appointments on the calendar.
Organize healthcare documents.
Inspect medication storage for cleanliness, organization, and expiration dates.
Review narcotics inventory sheets.
Order medications that have only seven days remaining.
Conduct bi-weekly orientation training as scheduled.
Complete monthly nursing summaries and reconciliations.
Review medication exchanges at assigned locations.
Develop and update a Medication Support plan as necessary.
Qualifications:
Degree from an accredited nursing program OR current and valid Registered Nurse license in good standing.
Valid driver's license from state of residence.
Must be at least 18 years old with a minimum of one year of nursing experience; supervisory experience preferred.
Minimal travel may be required to designated operations/locations.
