As a Pediatric Speech Language Pathologist - Homecare Specialist at Intermountain Health, you will provide quality speech language pathology services to patients and families in the home, ensuring timely and thorough documentation of patient records consistent with agency standards. This role requires the ability to practice within the Scope of Practice and comply with all policies, laws, licensure, regulations, and standards for care pertaining to the delivery of home care services.
Key Responsibilities:
• Provides timely and thorough documentation of patient records consistent with agency standards.
• Practices within the Scope of Practice and complies with all policies, laws, licensure, regulations, and standards for care pertaining to the delivery of home care services.
• Demonstrates competent theoretical and technical skills pertinent to patient populations being served, including Dysphagia, Speech/Language Pathology, Communication skills, Cognition, Voice, and Feeding skills.
• Demonstrates compliance with patient care coordination, including weekly and monthly chart audits as requested, utilization, infection control, improvement in corrective action plans, and attendance at staff and training meetings.
• Follows the patient care process, including thoroughly identifying and evaluating functional and rehabilitative needs, identifying and prioritizing patient needs, developing and implementing appropriate care plans and goals, adjusting and updating care plans as needed, and providing appropriate education to patients and families.
• Demonstrates the efficient use of resources by effectively managing time, supplies, and equipment and by meeting established productivity standards.
Requirements:
• Valid/current Utah Speech Language Pathology License.
• Current driver's license, insured, reliable transportation, and acceptable driving record.
• Basic Life Support Certification (BLS) for healthcare providers.
• Ability to access the Internet or a phone line transferring patient files twice a day.
• Effective communication skills.
• Basic computer skills.
• Current certificate of clinical competence.
Preferred Qualifications:
• Infant feeding experience.
• 1 year experience.
Physical Requirements:
• Hearing/Listening, Manual Dexterity, Pulling/Pushing Seeing, Speaking.
