We're seeking a skilled and compassionate Registered Nurse to join our team in Holyoke, MA. As a Home Health RN with LHC Group, you'll have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Supervise and provide guidance to home health aides and licensed practical vocational nurses, ensuring high-quality patient care.
- Provide comprehensive assessments, including medication reconciliation, and accurately document patient visits.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in on-call rotation and weekend rotation as needed to meet patient needs.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
