Job Title: Population Health Pharmacy Technician

We are seeking a highly skilled and motivated Pharmacy Technician to join our Population Health team at Trinity Health. As a Pharmacy Technician, you will play a critical role in supporting our Population Health initiatives to improve patient care and outcomes.

Job Summary:

The Population Health Pharmacy Technician will assist the Pharmacist in monitoring and educating patients on medication adherence and disease management to ensure their health goals are met. This role will also involve gathering information from medical records, Excel spreadsheets, and other technology-based systems to support quality measure performance.

Key Responsibilities:
  • Assist the Pharmacist in preparing prescriptions and performing various material resource management, dispensing, clerical, and billing duties.
  • Monitor and educate patients on medication adherence and disease management to ensure their health goals are met.
  • Gather information from medical records, Excel spreadsheets, and other technology-based systems to support quality measure performance.
  • Transport medications to patient rooms or nursing medication rooms.
  • Retrieve unused medications from patient rooms or nursing areas.
Requirements:
  • Current and Valid Idaho Board of Pharmacy Technician licensure in good standing.
  • Excellent verbal and listening skills on the phone.
  • Ability to utilize Excel and other Microsoft programs.
  • Able to use pharmacy information and gather patient data on EPIC EMR (or willing to train / learn efficiently).
  • Ability to establish and maintain cooperative and collaborative working relationships with pharmacists, providers, nurses, and other clinical staffs.
  • Skill in working independently and following through with minimum direction.
  • Ability to set priorities which accurately reflect the relative importance of job responsibilities.
  • Demonstrate writing skills to compose logical, detailed and grammatically correct correspondence via electronically.
  • Knowledge of efficient interviewing practices and techniques used in order to gather information from patients, pharmacy staffs and medical staffs.
  • Strong interpersonal skills with the ability to build relationships with patients and medical staffs. Excellent general customer service skills.
  • Knowledge of medical records and appropriate documentation requirements.
  • Ability to remain calm and be resourceful in stressful situations over the phone.
  • Ability to maintain a professional demeanor at all times.
Preferred Experience:
  • Prior experience in Medication Adherence Programs.
  • Experience with EPIC EMR functionality. Strong telephonic communication skills.
  • 1 year experience preferred as a pharmacy technician.
  • National Certification (CPhT, ExCPT or equivalent) preferred with under 3 years' experience.
Work Environment:

This position will be a hybrid or remote opportunity. The Pharmacy Technician in this role will not work weekends, nights or holidays.
