Home Health Speech Language Pathologist

Summit Care is a leading healthcare provider offering quality care services to residents across the country. We are currently seeking a dedicated Home Health Speech Language Pathologist to join our team of professionals. In this role, you will work closely with our multidisciplinary team to provide comprehensive speech therapy services to patients in their homes.
Key Responsibilities:
- Provides speech, language, swallowing, cognition, or voice therapy evaluations, screenings, and treatments to meet the needs of patients.
- Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice.
- Ensures execution of doctor's orders and maintains accurate patient records.
Requirements:
To be successful in this role, you will need a valid Speech-Language Pathology License to practice in appropriate state(s), as well as effective written and verbal communication skills and leadership and customer relations skills. Our ideal candidate will have experience working in home health settings and be familiar with electronic medical records.
Salary Range:
$80,000 - $110,000 per annum, based on experience, plus benefits and opportunities for professional growth and development.
